Chronic disease management
Nearly 50% of Australians suffer from one or more chronic diseases. Our Accredited Exercise Physiologists are specialised in chronic disease management and will be able to help you stay or become active, in a safe and comfortable environment.
They will take every condition into account and prescribe an individual tailored program that suits your schedule as well as your short- and long term goals.
With a chronic condition you will be eligible for allied health services that are partially or fully covered by Medicare. Each calendar year you can receive 5 allied health visits. If you are of Aboriginal or Torres Strait Islander descent, you are eligible for an additional 5 visits per calendar year.
Additionally, if you have type 2 diabetes, Medicare covers an initial consultation and 8 group sessions per calendar year.

How do I get a care plan?
- Contact your GP to book in for an appointment to discuss your eligibility for a care plan (phone consult or face-to-face).
- Your GP will fax or email the care plan to Phoenix Health & Fitness.
- When the care plan has been received, you will be contacted within 2 business days if you haven’t already contacted us.
